Tom Aspinall’s long-awaited return at UFC 321 was supposed to reset the heavyweight division. After spending more than a year waiting on clarity around Jon Jones, he finally stepped back into the Octagon against Ciryl Gane in Abu Dhabi. Instead of momentum, he walked into another major setback.
Aspinall suffered not one, but pokes to both eyes. Watching it live was painful. The fight lost rhythm immediately, and what should have been a statement performance turned into concern.
Now we know how serious it was.
Aspinall recently underwent surgery on both eyes and shared the aftermath on his Instagram story, with the image captured by Happy Punch on X. The visual was gruesome. Both eyes were completely bloodshot — deep red, almost glowing. The whites were overtaken by visible trauma, with swelling around the sockets. It didn’t look like irritation.

Optical specialists at Optegra later confirmed their involvement, stating: “After his highly publicised eye poke injury, Optegra’s expert team is proud to have supported MMA fighter and UFC Heavyweight Champion @tomaspinallofficial. Over recent months we’ve been working closely with him on his recovery. Following his operation on both eyes we continue the journey to get Tom’s vision back to fighting fitness.”
That statement ends the skepticism. Some fans questioned the severity immediately after UFC 321. But surgery on both eyes — and months of specialist care — tells the real story.
This isn’t minor. It’s a serious setback.
Aspinall is now expected to be out for an extended period as he works toward full visual recovery. For a heavyweight who already endured a long layoff, the timing couldn’t be worse.
Another pause. Another reset.
