Extreme weight cutting has long haunted the sport of MMA like a recurring nightmare. Voices such as Joe Rogan have repeatedly called for the practice to be eliminated to protect fighters. The dangers of this were on full display during Brian Ortega’s brutal cut ahead of his UFC Shanghai co-main event against Aljamain Sterling. In a non-title featherweight clash, ‘T-City’ missed weight by nearly 7 pounds and appeared visibly drained at the weigh-ins. The fight itself was nearly scrapped before both sides agreed to proceed at a 153-pound catchweight. Yet, as Ortega has recently revealed, the situation was far more alarming than it appeared, shedding light on the severe health complications he endured just to make it to the event.
Given how visibly drained Brian Ortega appeared before the fight, many fans believed the UFC would step in and prevent the former featherweight title challenger from competing. Shockingly, that wasn’t the case, and the bout went ahead at catchweight. What followed was a one-sided affair, with ‘T-City’ ultimately dropping a unanimous decision.
Recently, however, Ortega took to social media to share the harrowing reality of his weight cut, revealing that he nearly lost consciousness for 30 minutes in the process.
He said, “We cut weight on Thursday, we hit the first one. We got down to ‘[151 pounds]. Second one, later on, I decided to jump the gun on that one at 12. We cut from midnight all the way until 8 in the morning, cutting weight the entire time. I realized I had 1.7 [pounds] to go still.”
At that point, Ortega revealed his body simply stopped responding. “We were confused on how my body wasn’t really pouring out the water, why it was just holding it in, but no matter what we have to get this weight off, so we did. We went downstairs at about 8, 8-something, decided to cut more weight. Put the plastics on, Sweet Sweat, the whole thing. I did 20 minutes on the bike.”
That final session proved to be disastrous. Brian Ortega collapsed and was left unconscious for nearly 30 minutes. “Once I got off, I went unconscious. I was unconscious for about 30 minutes. During that time, they were putting ice on me. They took all my clothes off and left me in boxers. Woke up in the ER.”

Despite this frightening episode, Ortega still showed up for the weigh-ins. His team was prepared to call off the fight if his condition worsened, but ‘T-City’ pushed forward. He even shared that Sterling told him not to worry since the UFC already had a replacement lined up. Still, Ortega insisted on going through with it.
The former title challenger admitted his body was screaming at him to stop, but he chose to compete anyway, for his family and the fans.
“First and foremost, I decided to fight for my family. That’s my job, to show up and do what I do for them. I fought for my family. Secondly, I fought for you guys. You guys have always shown me love, you guys have always supported me, no matter what. It would be unfair to not show up for you guys, regardless of excuses, you just show up and do what you’ve got to do.”
This situation highlights the dangerous reality of extreme weight cutting in MMA. Fans will remember T.J. Dillashaw’s brutal cut to 125 pounds for his title fight against Henry Cejudo, a move that severely hurt both his health and performance. Similarly, Max Holloway’s skeletal appearance during his attempted cut for UFC 223 against Khabib Nurmagomedov forced doctors to pull him from the bout entirely.
More recently, fighters like Ilia Topuria and Islam Makhachev have moved up divisions after realizing their bodies could no longer handle the demands of cutting to 145 and 155 pounds, respectively.
As for Brian Ortega, he has also considered a move to lightweight, even teasing the possibility ahead of his fight with Diego Lopes at Noche UFC in September 2024. Now 34 years old, Ortega’s body is no longer able to endure the same drastic cuts he managed in his prime.
It’s been a tough stretch for the former title challenger. Once an undefeated 14-0 contender who rode into his first UFC championship bout against Holloway in 2018, Brian Ortega has struggled to regain that momentum. He is currently just 2-5 in his last seven fights, underscoring the challenges he has faced both inside and outside the cage.
