GLENDALE, Ariz. Cardinals quarterback Kevin Kolb was taken out of the game and his return was in question Sunday after a hit to the head in the first quarter of Arizona's game against the 49ers. San Francisco defensive tackle Justin Smith sacked Kolb, who fumbled and then took a knee to the head from San Francisco linebacker Ahmad Brooks. Kolb was taken to the locker room shortly thereafter, and backup John Skelton entered the game on the Cardinals' next possession. Kolb returned to the sideline late in the quarter. Kolb returned to the lineup last week after a four-game absence because of an injured right foot.
