Fox Sports:
Green Bay Packers safety Nick Collins sustained what is believed to be a head/neck injury early in the fourth quarter at Bank of America Stadium and was removed from the field on a stretcher.
The injury occurred with 12:15 remaining, when quarterback Cam Newton completed a 12-yard pass to Jonathan Stewart. Collins appeared to be struck in the helmet by Stewart's knee during the tackle and was down on the field for several minutes.
Collins, 28, waved to the crowd as he was wheeled off the field. He was declared out for the remainder of the contest.
